What is the clearest result of poor line hierarchy in a composition?

The governing concept is hierarchy: visual emphasis should help the viewer distinguish what is primary, supporting, and distant. Artists create this order through differences in line weight, contrast, density, sharpness, detail, and placement. When these differences are poorly controlled, the eye receives competing signals and cannot easily decide where to look first or which forms matter most. Option A is correct because poor hierarchy produces confusion among focal, secondary, and background areas. Option B describes the opposite of the problem. Line hierarchy does not automatically improve colour, and it cannot change the physical texture of the paper. The issue is organisation of attention within the composition.
